Sliding Scale Fees:

Sliding scale fees are based on family size and income. This program offers anywhere from a 10% discount to a 50% discount. We need proof of Income for one month from all members in your household that receive income, to determine eligibility. Payment is required at the time of service to receive the discount. No-cost services are available for well-child health exams and family planning. Other grant funds may be available as well. Please check with our receptionist if you think you might be eligible for any of these discounts. Patients who do not qualify for the discount program and remain on self pay will be offered a 10% discount for payment in full at the time of service.

No one will be denied access to services due to the inability to pay.

We participate with many insurance plans, including Medi-Cal, Medicare, Blue Cross, Blue Shield and Delta Dental. If your insurance covers treatment, you will receive the benefit of reduced personal costs. If your Insurance has a Co-pay we will require that you pay it at the time of service. Insurance policies vary, so we will review your insurance to determine the appropriate course of action. Once treatment has started, we will file your claims. If you have a question about what Insurances our office will accept please feel free to call our office and talk to the Insurance Specialist. We will do our best to become a contracted provider with your insurance if this is possible.

To help us with your insurance filing, please complete the insurance questions presented to you at the time of your appointment. Please remember that insurance is a benefit for you or your family. If, during the course of treatment, your benefits change, the financially responsible party is accountable for all charges. We request that you let our office know of any changes with your insurance as soon as they take place or at your next scheduled appointment.
